Office Forms

Below you will find several commonly requested alumni forms. If you have difficulty viewing a form in your browser, right-click on the link and select "Save as...". Save the form to your computer and open it in a PDF viewing program (like Adobe Acrobat Reader).

Alumni Audit Form

Alumni1 of Wheaton College are able to audit up to two courses per semester on a space available basis after credit students have registered. The alumni audit fee is $50 per course, plus course fees, if applicable. Alumni who wish to take advantage of this benefit must complete the alumni audit form. Signatures are required of the course instructor and from a staff member of the Alumni and Parent Engagement office. The completed form should be turned in to the Office of the Registrar, Student Services Building, 2nd floor. You may choose to be billed or make payment at the time of registration.

1 Students are considered alumni of Wheaton College once they have completed 30 credit hours (1 year) of undergraduate coursework or 1 semester of graduate coursework.

 

Forms for Faculty, Staff, and Students

***Please note: Please submit these forms a minimum of two weeks before the desired send date, along with the finished version of your mail/email piece. Our staff will be in touch with you as soon as it is received. Solicitations of any kind may not be sent in the month of June, or from Thanksgiving to New Year's.

Email Request Form

If you need to email alumni, fill out the email request form. Please be sure to include a finalized copy of the email you'd like to send. Processing time for email requests is two weeks from the date the request is approved by Alumni Engagement.
