Below you will find several commonly requested alumni forms. If you have difficulty viewing a form in your browser, right-click on the link and select "Save as...". Save the form to your computer and open it in a PDF viewing program (like Adobe Acrobat Reader).

Forms for Alumni

Audit Form

To audit a course for a reduced fee, print an alumni audit form, then stop by the Alumni Office for an approval signature. You may audit several classes at once, but you will need a separate form for each class.

Biographical Update Form

If you have a new address, or wish to make other changes to your alumni record, you can do that right here on the alumni website! Simply register (or log in if already registered), and the click on Update My Info in the "Member Tools" menu.

Forms for Faculty, Staff, and Students

***Please note: Please submit these forms a minimum of two weeks before the desired send date, along with the finished version of your mail/email piece. Our staff will be in touch with you as soon as it is received. Solicitations of any kind may not be sent in the month of June, or from Thanksgiving to New Year's.

Mailing Address Request Form

To order alumni address lists or labels, fill out the address request form. Please be sure to attach a copy of the material you are sending.

Email Request Form

If you need to email alumni, fill out the email request form. Please be sure to include a finalized copy of the email you'd like to send. Processing time for email requests is two weeks from the date the request is approved by Alumni Engagement.

Senior Scholarship Application

Every year, the Alumni Association awards merit scholarships to a select number of juniors in their senior year. If you would like to apply, download and fill out the senior scholarship application. Completed applications must have been submitted to the Student Development Office (SSB 218) by Wednesday, October 28, 2015. Award winners will be announced at spring Convocation. Applicants will be chosen on basis of academic achievement, campus/community involvement, spiritual growth, understanding of life mission, and future plans.
